Consultation Privacy Notice

This notice explains the information collected through Lee-Anne Wine’s consultation request forms. Please submit only a brief topic description, without detailed sexual or medical history or identifying details about other people.
What the request forms collect
The forms ask for your name, email address, country and region of residence, your age in years, confirmation that you are at least 18, a brief topic description, and your acknowledgement of the consultation guidelines. A request is not a booking or a payment.
Why the information is requested
Your information is used to review suitability and location eligibility, respond to your request, and, if accepted, arrange the consultation. A change in location may require a new eligibility review. Submitting a request does not subscribe you to marketing emails.
WordPress storage and email notifications
Requests are processed through this WordPress website’s Jetpack forms. Submitted answers are saved in the website’s form responses and emailed to Lee-Anne’s designated Gmail inbox. People with authorized access to those accounts may be able to access the information. WordPress/Jetpack and email providers process information through their services, including spam prevention. Their processing may involve locations outside your country.
Please keep your initial description brief. This form is not intended for detailed clinical intake or urgent concerns. If further information is needed after acceptance, Lee-Anne will explain what is needed and how to provide it.
Payment information
No payment is collected through the request forms. After acceptance, you will receive separate Stripe payment instructions. Stripe processes the information you provide at checkout. Do not enter card or banking details in the request form.
Access, retention, and privacy requests
Submitted request records and related email correspondence form part of the consultation administration records. Retention and deletion must take account of the purpose of the record and applicable professional and legal obligations. You may contact Lee-Anne to ask about access, correction, deletion, applicable retention periods, or how your information is handled. Deletion may be subject to recordkeeping obligations.
To make a privacy request, use the website’s contact form and label your message “Privacy request,” or reply to a consultation email. Do not include sensitive history in that message. If you decide not to proceed, you may contact Lee-Anne about your submitted information.
